iHOP Call for Proposals

The Passport Health Plan Improved Health Outcomes Program (iHOP) goal is to encourage innovation and research that can improve health outcomes for uninsured and low-income Kentuckians receiving Passport Health Care services. The program will provide grant support for research and demonstration projects.  Providers, clinicians, researchers or any 501c3 public charities may apply for funding.

Passport Health Plan has allocated $500,000 for the 2008 funding cycle.  Applicants may apply for up to $50,000 in grant funding.
