iHOP Frequently Asked Questions

What is the goal of Passport Health Plan's Improved Health Outcomes Program (iHOP)?
The iHOP goal is to encourage innovation and research that can improve health outcomes for Passport Health Plan members and uninsured individuals in our 16-county service area. The program will provide grant support for research and demonstration projects to accomplish this.

Which counties are served by Passport Health Plan and must iHOP award recipients carry out their iHOP initiative in these counties?
Passport Health Plan serves the following counties: Jefferson, Oldham, Trimble, Carroll, Henry, Shelby, Spencer, Bullitt, Nelson, Washington, Marion, Larue, Hardin, Grayson, Meade, and Breckinridge.  All award recipients must be located in and do their iHOP work in these counties.

Who may apply?
Providers, clinicians, researchers or any 501c3 public charities may apply for funding.

What is the area of focus for funding grant projects?
All grant applications focusing on improving health outcomes for Passport members or related populations will be considered.  Projects that address quality of care, access, satisfaction, cost effectiveness, or a better understanding of the health needs of this population are particularly important.  Unique partnerships or collaborations beyond the traditional silos of care are encouraged.

How often will Request for Proposal's (RFP's) be issued? 
RFP's will be issued annually.

What is the application deadline for this cycle?
Letters of Intent must be received by TBD.   Applicants will be notified by TBD if they were selected to submit full applications which must be received by TBD.  Final award decisions are begin TBD, with funding periods commencing TBD.

How many grants will be awarded per year? 
The number of grants awarded per year will depend on the amount of funding available per annum and the quality of the applications received.  

What is the minimum and maximum size of grant awards? Are applicants limited to one application per cycle?
There is no minimum award size; however, awards will be limited to a maximum of $50,000 per applicant.  There is no limit on the number of applications per organization.

How long is the grant period?
Grant periods are one-year in length, but no-cost extensions may be granted.

What is the process for submitting applications?
All interested applicants must issue a Letter of Intent, not to exceed one page, to the iHOP program at the address below by the above deadline.  Those applicants selected to submit a full application will be notified by TBD and sent full application templates and instructions.  The iHOP staff will be available to guide and advise you through the process.

What is the review process?
Upon receiving appropriately completed applications, the iHOP program staff will issue grant applications to the iHOP Review Panel.  The Panel will advise on decisions on a grant by grant basis to determine as to which projects merit consideration for funding by the iHOP program.

Is there a mechanism for applying for a second year of funding?
Applicants may reapply for a second year of funding if there is sufficient cause to indicate a requirement for project success.  However, no priority will be given to previous recipients when reviewing applications.

May the grant budget include indirect costs?
Yes.  Grants may include a maximum of 10% indirect costs in the line item budget (see line-item budget template).  Total budgets (direct and indirect expenses) may not exceed $50,000.

May iHOP funds be used for travel expenses or computer/software purchases?
Only travel expenses directly related to carrying out the specific project may be included.  Travel to national meetings is not permitted for example.  Computers and software are not allowable iHOP expenses.

Do you provide feedback to applicants whose grant proposals are not funded?
No.  It is not our policy to provide formal feedback on denied applications but the iHOP staff are available for advice and direction.
